The first required documents that must be submitted in the application process are the Application and application/examination fee. The Board office will no longer hold required documents that have been submitted prior to receipt of the Application and the application/examination fee.
A notary public is available in the Board office. Only documents presented by the affiant will be notarized, the fee is $3.00 per document; the Board cannot accept cash.
